How often do water regulators fail?

I got a call to a walk in (cooler) with a water cooled condenser. No water pressure gauges on it. High pressure cuts out at 400PSI. R404A. Low side about 50psi. I let it sit for a bit to cool off. That water loop took a second but water was definitely going through it cause it was getting cold. Put that thing wide open as I could get it and got the high side to hang out around 350 psi. Once the cooler hit about 41 degrees it started cutting out again. Not too familiar with water cooled stuff. Didn't seem like any restrictions. Tees off to 2 evaporators. Neither were iced up. Almost seems like it might be overcharged too?

Do water pressure regulators fail often? The cooler was working before. It's about a year old.
